Transition Coordination services involve working as part of the Transition team with students aged 14-22 receiving special education services, their families, teachers, related services, community agencies and area businesses. Our goal is to collaborate through multi- agency planning and partnerships in order to help accomplish successful adult outcomes for our students within the areas of education, employment and independent living.
